Service Authorization & Liability Waiver

I confirm that I lawfully possess the submitted firearm or components and am authorized to approve the requested work. I authorize Creative Freedom to perform the services described in my approved work order.

I understand that milling, machining, barrel porting, Cerakote, and other authorized modifications may permanently alter my firearm beyond its original manufacturer’s specifications and may affect operation, reliability, compatibility, and manufacturer warranty coverage. I voluntarily accept the risks associated with these modifications and the subsequent use of the modified item, including property damage, serious injury, or death.

To the fullest extent permitted by applicable law, I release Creative Freedom and its owners, employees, and agents from claims arising from the authorized modifications or the modified item’s subsequent use or misuse. This release does not apply to gross negligence, reckless or intentional misconduct, or liability that cannot legally be waived.

I agree to indemnify and hold Creative Freedom harmless from third-party claims and reasonable expenses, including attorneys’ fees, to the extent caused by my negligence, misuse, unlawful conduct, or subsequent alterations.

Shipping: For The Chevron package, do not send your entire handgun unless approved for full-handgun Cerakote or otherwise authorized in writing. Creative Freedom requires firearms, including frames/receivers, to arrive unloaded in a locked container. This locked-container requirement does not apply to shipments containing only a slide, barrel, and recoil assembly. No live ammunition.
